- Summit Medical Group does not have a contract as a participating group with Horizon Blue Cross/Blue Shield. We will submit your claim form on your behalf and you should be reimbursed directly by your plan per your out-of-network benefits. We recognize you have additional out-of-pocket expenses when using an out-of-network provider and certainly appreciate your patronage of Summit Medical Group.
