Summit Medical Group is pleased to welcome geriatrician John M. Heath, MD, AGSF, to its medical team. As head of Geriatric Services, Dr. Heath will build a much-needed comprehensive geriatric program at the Group. In addition to seeing patients at the Berkeley Heights campus beginning in November, Dr. Heath will see patients at Care One in Livingston, New Jersey. He also will avail himself for geriatric transitional care hospital consultations and develop a home visit program for SMG house-bound and hospice patients.
Dr. Heath provides primary and consultive geriatric care for older adults. He partners with practitioners, patients, and patients’ families in coordinating care. In addition to his position at the Group, Dr. Heath is a clinical professor at Robert Wood Johnson Medical School (RWJMS) in New Brunswick. He has been a medical director for subacute rehabilitation, hospice, and home care organizations, and he has been Corporate Medical Director for the Care One senior care facilities throughout New Jersey. Dr. Heath helped initiate a palliative care service at Robert Wood Johnson University Hospital. He has been a member of the Carrier Clinic Foundation and Saint Peter’s University Hospital medical staffs. Before moving to New Jersey in 1996, he practiced geriatric medicine as a faculty member at the State University of New York (SUNY) Upstate Medical Center in Syracuse.
After earning his BS in health science at Gannon University and his MD at Hahnemann University School of Medicine, Dr. Heath completed his residency at Saint Vincent Health Center and fellowship at the University of Cincinnati. He is board certified in geriatric medicine.
