Heart failure is a condition in which the heart muscle weakens and cannot pump blood effectively. It results from high blood pressure, heart attack, valve disease, or other health problems. Symptoms of heart failure include shortness of breath; feeling tired or weak overall; swelling in the feet, ankles, and legs; weight gain from the build up of fluid; cough; and loss of appetite. Fortunately, there are effective treatments for heart failure.

A team of medical experts, including your physician, cardiologist, physician assistants, and registered nurses will partner to help strengthen your heart and improve your tolerance for activity. In 5 structured sessions, we will teach you about your heart, how to keep it strong, and how to protect it from unwanted problems.
Your first session will last approximately 1 1/2 hours. All other appointments will typically last an hour or less. Our physician assistants also will closely monitor how you respond to your heart medications. Once you have completed your sessions, your cardiologist, the clinic staff, and our nurse case manager will continue to follow your care so that you get the treatment you need to protect your heart health.
We also invite you to attend patient education luncheons with other patients from the Heart Failure Clinic. The luncheons are a good opportunity to get and give support with other people who are recovering from heart failure.
Spouses, significant others, and family members are welcome to join you at your clinic visits.
To register for our program, you must have a written prescription from your primary care physician or cardiologist.
