Veronica Grysko-Sporer, MSW is a Social Work Care Navigator for Summit Medical Group. Ms. Grysko-Sporer provides patient navigation services through screening and connection to appropriate care management and community services with the main goal of helping patients remain safely in their preferred home environment. She is trained and experienced in guiding patients and family members through the health care system and potential crisis situations by conducting assessments, intake screenings, and offering appropriate referral services as needed.
Ms. Grysko-Sporer’s experience includes providing individual assessments, treatment plans, crisis intervention, and psychotherapy services for adults with depression and other psychological disorders. She also has experience organizing and implementing programs which helped improve the quality of life for elderly residents. During her time as a cancer registrar, she compiled and communicated critical patient data to physicians for research studies.
She currently serves as the Morris County Co-Chair for NASW-NJ Chapter where she is responsible for the membership, development, organization of meetings and activities for unit members. She previously served as the Northwest Regional Representative on the Board of Directors for the National Association of Social Workers and served on the Morris County SART (Sexual Assault Response Team).
In her free time, Ms. Grysko-Sporer enjoys volunteering and giving back to the community. She has fundraised for a variety of different organizations, Dress for Success, Leukemia & Lymphoma and Community Soup Kitchen. She loves spending time with her family, traveling, photography and participating in outdoor activities such as hiking, skiing.
